Mitchell Trubisky Mentor Elementary Football Camp
The Mentor High School Football Program will be offering a 3-day football camp, June 21-23, 2021, for all elementary students entering grades 1 through 6. The fee will be $125.00 for each child attending the camp. Each participant will be given offensive, defensive, and special teams instruction as well as a camp T-shirt, a student pass to all home games, and a signed picture with Mitchell Trubisky. The Lake County Salvation Army is sponsoring the 2021 Back to School Supply Bash to provide school supplies to all Lake County children who may be in need of some assistance for the 2021-22 school year. The event is being held on Saturday, August 7, 2021 at the Salvation Army located at 69 Pearl Street, Painesville. This event is by appointment only. Introductory Girls and Introductory Boys Youth Wrestling Clinics:
The Mentor Girls Wrestling and the Mentor Boys Wrestling teams will both conduct separate two-hour introductory clinics; one for girls, Kindergarten to 8th grade, on July 17th, 2021 and one for boys, Kindergarten to 8th grade, on July 18th, 2021. Mentor Schools Childcare Provider Form
A Childcare Provider form must be completed each school year if you need your student(s) transported to/from a Board approved daycare for your school. This also includes students needing to attend a private daycare such as with a relative, neighbor, etc. We will only transport to/from the individual daycare in your school area.

A copy of the Mentor Schools Childcare Provider Form is attached for your convenience. Please either return by fax (440-255-4707), email (BurtDa@mentorschools.org
), or mail to Mentor Schools Service Building, 7060 Hopkins Rd, Mentor, OH 44060, Attn: Jen Skulski. The deadline to submit a Childcare Provider form in order to assign your student a bus for the first day of school is July 15, 2021. All forms submitted after this date may take up to 10 business days after the start of school to assign busing.
